‘The service introduced a person centred electronic care record system. Hand held devices enabled staff to capture information throughout the day about each person’s care, wellbeing and how they spent their day. The system also highlighted any concerns such as when a person hadn’t eaten or drunk much, so nursing and care staff could respond proactively to address them.’
