Body maps, wound care, review cycles, assessments, risks and care needs are all integrated to provide comprehensive, detailed and up-to-date care plans.
Carers can access up-to-date care plans with the touch of a button on the Care App, so residents' needs and risks are always known about and the care they receive is responsive and personalised.
Important information is automatically fed between risk assessments, initial assessment and care plans. And evidence of care feeds into the care plans so all staff knowledge is involved in reviewing care.
Informed care plans that are integrated with industry acknowledged assessment tools, providing a holistic assessment of needs, risks and outcomes.
Personalised care plan routines incorporate preferences for social and medical actions needed to support residents' goals.

What solutions does Care Planning give care homes?

With an electronic care plan, the information is just as secure but able to be accessed by every care worker easily from within the Care App on their handheld device. As care plans are no longer locked in cupboards, staff can update or remind themselves of residents’ needs and person-centred care requirements at any time.
