Leveraging tech to transform person-centred care

Improving Australia's residential aged care communities

The Aged Care Quality Standards are being updated to strengthen the focus on person-centred, accessible quality care for older people.

The commission has outlined the outcomes providers must achieve for person-centred care in the guidance.

Person-centred care: putting individuals at the heart of everything you do

Residential aged care communities across Australia can enhance the quality of care they provide by implementing a person-centred approach. This methodology is not simply a trend but fundamental to achieving better outcomes for the individuals in their care.

By prioritising residents' dignity and independence, organisations can foster a positive environment that keeps residents happy, boosts the community's reputation, and ensures adherence to essential regulatory standards.

What is person-centred care?

Everyone in aged care has the right to be safe, treated with dignity and respect, and receive high quality care and services

At its essence, a person-centred approach places the individual at the heart of all care decisions and actions. This approach shifts the focus from a purely clinical framework to one that actively prioritises the unique needs and preferences of each person receiving care.

Instead of relying solely on health and care professionals to make decisions on their behalf, residents are encouraged to participate in discussions about their care and treatment options. This fosters a sense of empowerment as they become active participants in their own care journey.

In practice, person-centred care involves several key components. Firstly, it emphasises shared decision-making, recognising the individual as an equal partner in the care process.

This means that providers engage with residents to understand their personal goals and values, ensuring that treatment plans align with what matters most to them.

Furthermore, the person-centred approach prioritises the individual's overall wellbeing by considering a wide range of factors that affect their health and happiness. This includes not only their clinical and medical needs but also their personal preferences, social interactions, and lifestyle choices.

Aged care workers can provide more tailored and responsive care by understanding each resident's unique background, needs, and interests. For example, care plans may incorporate the individual's favourite activities, dietary preferences, and social connections, promoting a holistic view of health that extends beyond traditional medical care. This kind of respectful and responsive caregiving ensures that individuals feel valued and understood, greatly enhancing their quality of life.

In summary, providing care that is both respectful and responsive—attuned to the desires, personal circumstances, interests, and lifestyles of each individual—guarantees that the person receiving care remains at the forefront of the overall care process

By embracing a person-centred approach, aged care communities can create an environment that truly supports the welfare and dignity of every resident.

How to leverage tech to transform person-centred care

Technology in the aged care sector is significantly transforming the delivery of care for older adults, making it more efficient and person-centred.

One particularly noteworthy innovation is the development of person-centred care software. This software enhances the quality of care in aged care communities by placing residents' needs and preferences at the forefront, allowing care workers to tailor their services accordingly.

At Person Centred Software, we are committed to providing aged care software that helps your organisation meet regulatory compliance standards, enhances the delivery of person-centred care, and improves resident outcomes.

Our clinical care system, mCare

For example, our innovative clinical care system, mCare (which enables aged care staff to evidence all aspects of care), is equipped with an extensive range of features designed to empower care staff to deliver a more person-centred approach to care planning.

  • The clinical care system's 'Person-centred Care Plans' feature helps care workers make person-centred care planning easier to implement and maintain. By leveraging this feature, care staff can ensure that each resident's needs are being met with the utmost attention and respect, which is essential for improving residents' overall wellbeing and satisfaction.

  • When using the clinical care system, aged care staff can truly personalise care with the 'Person-centred Interactions' feature. This feature ensures that all interactions are specifically centred on the individual receiving care, allowing for a more tailored experience. The system includes valuable features such as 'Who I Am' and 'Plan Care Day'. The 'Who I Am' feature allows care workers to gain deeper insights into the individual's background, preferences, hobbies, and any other important information that shapes their identity. This understanding enables staff to engage more meaningfully and respectfully with each person. The 'Plan Care Day' feature allows care workers to customise daily care routines based on the specific needs and desires of the individual. By planning care activities that align with the person's interests and preferences, staff can create a supportive environment that fosters autonomy and wellbeing.

  • With the 'Goal Tracking and Outcomes' feature, care providers can actively support residents' goals and keep track of their progress towards meeting them. Having this capability allows a person-centred approach to caring and fulfilling goals. For example, when an older adult is discharged from the hospital and transitions to an aged care site for the first time, the goal might be to ensure their initial day in the home is peaceful and comfortable, helping them adjust to their new surroundings in a supportive and nurturing manner.
