Secure access to resident’s healthcare information
Person Centred Software is continuously adapting its Clinical Care System to meet the needs of aged care providers across Australia. Partnering with industry experts and organisations that integrate and work alongside its Clinical Care System, Person Centred Software has created an ecosystem of care to equip aged care providers with the best-in-breed solutions for every part of their business.
The integration with Australia's national digital health record platform, My Health Record (MHR), managed by the Australian Digital Health Agency (ADHA), offers a number of important benefits for both aged care providers and residents.
What is My Health Record?
My Health Record is a way to provide a secure electronic summary of people's medical history which will soon include information such as medications, allergies and immunisation history.
Having a My Health Record allows healthcare and aged care providers to access people's health information securely online, anywhere, at any time – even if they're moving or travelling interstate. So, whether the resident is visiting a doctor for a check-up or in an emergency room following an accident and is unable to talk, healthcare and aged care providers involved in their care can access important health information.
What does the integration between My Health Record and the Clinical Care System do? And why is it important?
The integration between MHR and Person Centred Software's Clinical Care System allows a seamless transfer of accurate and up-to-date medical data between different organisations. For example, when a resident moves to and from a hospital setting, both the residential aged care facility and the hospital can benefit from quick access to all the latest care records, medical information, etc.
How will this integration benefit aged care providers?
Person Centred Software's My Health Record integration provides many benefits to aged care providers:
- Quickly and easily see residents' health information all in one place
- Facilitates person-centred care
- Enhances quality and safety of care
- Reduces time gathering clinical information
- Avoids duplication of services
- Improves medication safety
- Key health information is available in an emergency
- Those providing healthcare can access important data, leading to improved care pathways and outcomes
Connecting Aged Care Providers to My Health Record
Person Centred Software can identify individuals and organisations using the HI (Healthcare Identifier) service, a unique 16-digit number the My Health Record system uses to identify an individual. Once identified, Person Centred Software can connect aged care providers to the My Health Record system using the B2B gateway.
