About Care Consult ID
Care Consult ID provides outcome-focused supported living in the community for adults with learning disabilities, challenging behaviours, autism spectrum disorder, mental health needs and personality disorders.
Founded in 2022, the organisation also offers respite, home care and support services for older adults in the community. Its mission is to help people live full and meaningful lives in the comfort of their own home, with support that is built around each person’s needs, goals and independence.
To do that well, Care Consult ID needed care records that could support the reality of community-based care. Staff needed quick access to care plans, risk assessments, incident reports and daily notes while working with people in their own homes and out in the community. PCS Care (formerly mCare) gave the team a reliable way to record support, evidence progress and keep information close to the people providing care.
The challenge
Care Consult ID provides support 24 hours a day, including care delivered in the community. That means staff cannot rely on information being held in one place or only being easy to access from an office. Care plans, risk assessments, daily notes and incident records need to be available when staff are with the person they support.
Goals were also central to the way Care Consult ID wanted to work. For people with learning disabilities, short-term goals can make day-to-day progress feel clearer and more achievable, while long-term goals help people build confidence, independence and agency over time.
Care Consult ID needed a system that could support both sides of that work: the practical documentation required for safe care, and the person-centred records that show how someone is progressing towards the life they want.

Helping people work towards short and long-term goals
Care Consult ID uses PCS Care to set short-term goals and outcomes with the people it supports. That has been particularly useful for people with learning disabilities because it helps break progress down into clear, everyday steps. Goals can be discussed openly, recorded and revisited, so people can see what they are working towards and how far they have come.
Beatrice Nyamande, Registered Manager and CEO of Care Consult ID, shared an example of one individual who was visited by a social worker and was eager to talk about her short-term and long-term goals. She was able to refer back to past records and show the progress she had been making.
Clearer records for care in the community
PCS Care also supports the day-to-day documentation Care Consult ID needs to provide safe, consistent care. The team uses the system for care plans, risk assessments, report writing and incident reporting, helping staff keep important information together and easier to access. Staff can use handheld devices while working with people in the community, rather than waiting until later to update records.
That helps the team keep notes closer to real time and gives staff the information they need when they are supporting someone directly. For Care Consult ID, the value is practical: the system gives staff the tools to record care, understand needs and support people wherever care is being delivered.
Stronger compliance evidence when it matters
The assessment tools in PCS Care have also supported Care Consult ID’s compliance work. Beatrice highlighted accessible standards as especially important because the organisation supports people with learning disabilities and autism. The assessments help the team record more detail, learn from incidents and make referrals where needed.
Having information ready to share has made a clear difference. When the team recently needed to send risk assessment documents, staff could go into PCS Care, download the documents and send them quickly. They were also able to update the information before sending it, so the documents were as close to real time as possible.
The software is accessible and easy to use, and staff are able to access everything they need on handheld devices which never go wrong and never let us down.
