‘The provider had introduced an electronic care planning system, which was individualised and accurately maintained. Nurses and care staff were trained to us the system and each had their own linked hand held electronic tablet device, which they kept on their person. This enabled staff to access people’s care plan information, which included fast access to any advance care decisions they had made and to record any care given at or immediately following the point of delivery.’
