Joining up health and social care data

Through modern technology, information can flow easily between care homes and health services, such as GPs, hospitals and ambulance crews. By collaborating to share data securely and in a standardised way, service can provide a more joined-up approach to health and social care.

How PCS helps health and social care work better together

People move between health and social care services every day. When information doesn't move with them, it creates delays, duplication and unnecessary work.

That's why we've always worked closely with NHS organisations at national and regional level to improve how information is shared between care homes, GPs and other health and care services.

We were one of the first digital social care record providers to launch an integration with GP Connect, helping care homes access key GP information through PCS Care. Today, more than 1,000 care homes use GP Connect through PCS.

By connecting care homes with GP systems and other health technologies, we help reduce admin, improve communication and support better outcomes for residents.
