People’s care records included risk assessments, which were reviewed regularly and covered areas related to people’s health, safety, care and welfare. Care plans and associated risk assessments identified any changes in risks to people’s health and wellbeing. In most cases, we saw the care plans provided clear guidance to staff in respect of minimising risk.’
