CMM Magazine Digitising Transfer of Documents

'Person Centred Software and Sutton Council have worked on a demonstrator project as part of the South West London STP to digitise the transfer of documents between care homes and hospitals.

The aim is to improve the integration of information and communication when residents transfer between care homes and hospitals. So far, the trial has found that digitising the red bag process is time-consuming, but has reduced complaints and improved data protection and efficiencies.

Recommendations resulting from the trial include making the Data Security and Protection Toolkit more accessible and supporting care homes to receive a digital copy of discharge summaries from hospitals.'
